1 | Find out California's NEMT Requirements

To ensure compliance with California's NEMT requirements, you must fulfill specific criteria, including passing background checks, meeting driver and vehicle standards and adhering to regulations set forth by the California Department of Health Care Services (DHCS). They oversee Medicaid (Medi-Cal) and can provide all the guidelines and rules you need.

2 | Stay on top of federal NEMT regulations

NEMT services must also comply with federal regulations established by the Centers for Medicare & Medicaid Services (CMS). Ensure you are familiar with these federal requirements before launching your NEMT business operations.

3 | Get insured

To insurance-proof your NEMT business in California, reach out to specialized insurance providers offering reliable, legally compliant coverage. This shields your company, employees, and patient riders from accidents and unexpected events. While ITR covers some bases, securing multiple types of insurance guarantees complete peace of mind under the sunny skies of California.

4 | Obtain necessary licenses and permits

Check with local city and county offices, as well as the California Department of Financial Protection and Innovation on the necessary licenses for highly-regulated industries such as NEMT.

CaliGold is also a good resource that provides information on permits and licenses required for specific business types in California.

5 | Know your employer responsibilities

If you're considering hiring employees — and it's a good bet you will — thorough research is crucial. After all, who will be driving your extensive fleet? Even Bambi had his forest friends for support. How to Start an NEMT Business in California: Business Plan

How to Start an NEMT Business in California: A Several Part “Bambi Knows Best” State-By-State Series

Part 2: Develop a Business Plan and Get Registered in California

Based on your research into the California NEMT market, it's key to develop a thorough business plan. This plan is essential for establishing your business and accessing funding opportunities.

Begin by consulting resources like the California Secretary of State to understand the initial steps involved in starting a business in California. Key aspects include selecting a suitable business structure, completing registration, obtaining required licenses and permits and adhering to applicable tax regulations. The California Small Business Development Center (SBDC) is also a valuable resource for startup support.

Here's a concise guide to navigate this stage of launching your business:

1 | Choose a Business Entity

Decide on the legal structure of your business. Common options include Sole Proprietorship, Partnership, Limited Liability Company (LLC) and Corporation (C-Corp or S-Corp). Each has different legal and tax implications so be sure to understand the differences.

2 | Register Your Business Name and Entity

Upon choosing the biz entity that best suits your biz, the next step is to come up with a name for it. Have fun with this part of the creative process — we certainly did when we came up with our name, Bambi. Once you have a name, both the entity and the name must be registered with the California Secretary of State to conduct business in California or with Californians.

3 | Obtain an Employer Identification Number (EIN)

Apply for an EIN from the IRS if you will have employees or operate as a corporation or partnership.

4 | Register for State Taxes

In California, you can register for taxes and obtain necessary permits through the California Department of Tax and Fee Administration(CDTFA).

How to Start an NEMT Business in California: Market

How to Start an NEMT Business in California: A Several Part “Bambi Knows Best” State-By-State Series

Part 1: Research the California NEMT Market

Welcome to California, where the opportunities for launching your NEMT business are as golden as the state itself. Before diving into the intricacies of setting up your venture, conducting thorough market research is paramount. This initial step will enable you to navigate California's expansive landscape and varied communities effectively, ensuring you identify underserved areas and avoid entering oversaturated markets.

1 | Identify existing NEMT providers in your area

California has one of the largest Medicaid populations in the entire country, with over 14.2 million enrollees. To meet the non-medical needs of this population is support offered by Medicaid in California (Medi-Cal). Within their managed care health plans are NEMT services. The Department of Health Care Services is another great place to look for information on NEMT providers across the state. Here are some of the NEMT brokers and/or providers offering NEMT services today:

  • Modivcare: One of the largest NEMT brokers in the United States, managing transportation services for Medicaid and Medicare recipients across multiple states, including California.
  • MTM, Inc.: MTM provides NEMT services nationwide, including California, and works with government agencies, managed care organizations and healthcare providers to arrange transportation for patients.
  • American Logistics Company (ALC): Based in California, ALC offers a range of transportation solutions, including NEMT services. They partner with health plans, transit agencies and school districts.
  • CareLinx: This company provides in-home care and transportation services, including NEMT. They operate throughout California, offering personalized care solutions.
  • MedTrans: MedTrans is a regional provider specializing in NEMT services across California, focusing on safe and reliable transportation for medical appointments.
  • USF Health: Although not exclusively an NEMT provider, the university hospital connects healthcare providers with transportation services, including NEMT, to improve patient access to care.
  • Veyo: Veyo is now an MTM company  — as they were recently acquired — but nothing has changed in terms of their quality service, and Veyo is still able to operate pseudo-independently. In fact, their Independent Driver Provider (IDP) tech is driving this VeyoRide initiative
  • Uber Health: Provides non-emergency medical transportation services in San Diego, LA and other cities in California. They offer a HIPAA-enabled platform for healthcare providers to request and monitor patient rides.
  • Your favorite search engine. Of course, one can always perform their own search with the keywords, “NEMT providers in California” or get more granular with your search by typing in specific municipalities (i.e. “NEMT companies in San Diego”).

2 | Assess the demand for NEMT services

Embracing the golden opportunities for your NEMT business in the sunny California landscape starts by understanding the demand in your local area and deciding whether to serve multiple jurisdictions or focus within a specific radius. Dive into research on nearby hospitals, healthcare facilities, wellness centers and other health-focused establishments. Think outside the obvious and look beyond the usual methods to uncover unique opportunities that set your business apart.

  • Adult Day Care (ADC) Centers. California has a robust network of Adult Day Care centers to support its large and diverse senior population. For the latest statistics and information, refer to the California Department of Aging (CDA) or the California Department of Social Services (CDSS). These agencies oversee and regulate adult day care centers and provide databases or lists of licensed facilities throughout the state.
  • Dialysis Clinics. While specific data points are unavailable, California's vast and diverse population ensures numerous dialysis clinics are located across the state. Some of the largest clinics with a significant presence include Fresenius Medical Care, DaVita Kidney Care and U.S. Renal Care.
  • Physical Therapy Centers. Due to diverse reporting methods and California's extensive healthcare network, obtaining an exact number of physical therapy centers can be challenging. For the most accurate information, we recommend consulting the California Department of Public Health (CDPH) records, professional associations such as the California Physical Therapy Association (CPTA), or industry reports focusing on California healthcare facilities.
  • Hospitals. California has approximately 416 hospitals. This includes both general acute care hospitals and specialized facilities. For the most accurate and up-to-date information, you can refer to the California Office of Statewide Health Planning and Development (OSHPD), which maintains detailed records on the state's healthcare facilities.
  • Rehabilitation centers. We don’t have an up-to-date number on these, but many hospitals partner with rehab centers so it’s always best to contact those 416!
  • Nursing homes and assisted living facilities.  California has many nursing homes and assisted living facilities to cater to its elderly population. There are approximately 1,230 nursing homes and over 7,000 assisted living facilities in the state. These facilities provide long-term care services, including skilled nursing care and assistance with daily living activities, for elderly residents who require ongoing medical and personal care. For the most up-to-date information, consult the California Department of Public Health (CDPH) or the Centers for Medicare & Medicaid Services (CMS).
  • Outpatient care centers. Similar to rehab facilities, California boasts numerous outpatient care centers statewide. For more details on these estimates, consider consulting your local county’s hospitals.

3 | Decide on a target market

Upon thoroughly researching the NEMT market opportunities to learn about the demand across the state, it’s time to identify the most viable demographic to target — whether seniors, individuals with disabilities or a broader patient population. Honing in on a targeted group allows you to tailor your services and marketing strategies ever more efficiently.

4 | Choose a specific NEMT service (or offer them all!)

Dive into the California NEMT service wave by focusing on the one (or two) most in demand to effectively meet your community's needs. This strategic approach ensures a solid foundation and sets the stage for long-term success. Keep an eye on competitors and stand out by offering unique services or targeting underserved niches. Launch with a top-tier fleet designed for your specific area, whether transporting individuals to adult day cares, physical therapy centers or other vital destinations.

5 | Understand the different Medicaid delivery models

In California, Medi-Cal oversees NEMT services, ensuring they're available when a Medicaid recipient's medical condition makes public or private transportation impractical. This policy means only those meeting specific criteria can access NEMT assistance. Public transportation is considered a typical option, which affects the program's size and reach. Medi-Cal assumes existing services can handle most Medicaid transport needs, impacting both fee-for-service and managed care enrollees across 30 counties. Health plans also adhere to these guidelines when authorizing medical transportation for Medicaid recipients.

Here are some fast California facts according to the report State-by-State Profiles for Examining the Effects of Non-Emergency Medical Transportation Brokerages on Transportation Coordination:

  • NEMT Model is a blend of In-House Management and Managed Care Organization
  • The Operating Authority is the 11902(a)(70) State Plan Amendment
  • NEMT expense is classified as a mix between being a “Medical Expense” and an “Administrative Expense”
  • 15% live below poverty (US Census 2015 5-Year Estimates)
  • 90% live in Urbanized Areas (US Census 2010)
  • $136.1 Million is the estimated annual NEMT 2013 expense (Source 2014 National NEMT Survey)
  • 4,823,202 is the 2013 estimated number of annual NEMT trips

6 | Getting Paid

In California, NEMT providers receive payment through several methods under the Medi-Cal (California's Medicaid program):

  1. Fee-for-Service (FFS): Providers submit claims directly to Medi-Cal for each NEMT service provided. Reimbursement rates are determined by Medi-Cal and vary based on the type of transportation service and other factors.
  2. Managed Care Organizations (MCOs): California has a managed care model where MCOs are contracted by Medi-Cal to manage healthcare services, including transportation. NEMT providers contract with these MCOs to deliver services and receive payment according to negotiated terms and rates set by the MCOs.
  3. Brokers: Some MCOs or third-party brokers may coordinate NEMT services for Medi-Cal beneficiaries. Providers can contract with these brokers and receive reimbursement based on agreed-upon terms and rates.

Providers in California must comply with Medi-Cal billing guidelines and procedures when submitting claims for reimbursement. For detailed information and specific requirements, providers should refer to the California Department of Health Care Services (DHCS), Medi-Cal Managed Care Division or their contracted MCO.
By thoroughly researching the California NEMT market, you'll lay a solid foundation for launching and scaling (and surfing) up your business. Stay tuned for Part 2, where we'll guide you through crafting a comprehensive business plan tailored to California's regulatory framework and market dynamics. NEMT vs Uber & Lyft: Get the Competitive Advantage Over Rideshare

NEMT vs Uber & Lyft: Get the Competitive Advantage Over Rideshare

Is Rideshare the Competition for NEMT? Understanding the Landscape and Opportunities

As Non-Emergency Medical Transportation (NEMT) providers, it's crucial to understand the evolving landscape of healthcare transportation and the role of rideshare companies like Uber Health and Lyft Healthcare. While these services have made inroads into the healthcare transportation sector, NEMT providers should view this development as an opportunity for growth and innovation rather than a threat.

The Rise of Rideshare in Healthcare

Uber Health and Lyft Healthcare have indeed entered the healthcare transportation arena, offering convenient and often cost-effective solutions for patients needing rides to medical appointments. However, it's important to recognize that these services primarily cater to a specific segment of patients with basic transportation needs.

Key Advantages of NEMT Providers

NEMT providers possess several competitive advantages that set them apart from rideshare companies:

  1. Specialized Care: NEMT providers offer "door-to-door" or "door-through-door" service, assisting patients in and out of vehicles and through doorways. This level of care is essential for patients with mobility limitations or special needs.
  2. Trained Personnel: NEMT drivers are trained and certified in patient assistance, CPR, first-aid, and wheelchair securement. This specialized training ensures patient safety and comfort throughout the journey.
  3. Equipped Vehicles: NEMT providers operate specialized vehicles equipped with wheelchair ramps and securement systems, catering to patients with diverse mobility requirements.
  4. Insurance Coverage: NEMT services are often covered by Medicaid or private insurance for eligible patients, making them a more accessible option for many individuals.
  5. Regulatory Compliance: NEMT providers are well-versed in healthcare regulations and compliance requirements, ensuring that all transportation services meet necessary standards.

Leveraging Technology for Competitive Advantage

To maintain and enhance their competitive edge, NEMT providers should embrace technological advancements:

  1. AI-Powered Routing: Implement AI algorithms to optimize transportation routes in real-time, minimizing travel times and enhancing patient satisfaction.
  2. IoT Integration: Utilize IoT devices and sensors to monitor vehicle performance, leverage predictive maintenance, and reduce downtime.
  3. Telemedicine Integration: Partner with telemedicine providers to offer complementary services, such as remote monitoring and prescription refills.
  4. User-Friendly Platforms: Develop intuitive booking platforms and mobile apps to improve accessibility and user experience.

Collaboration Over Competition

Rather than viewing rideshare companies as direct competitors, NEMT providers should explore opportunities for collaboration:

  1. Hybrid Models: Consider partnering with rideshare companies to offer a comprehensive range of transportation options, catering to patients with varying needs.
  2. Focus on Specialization: Emphasize and expand specialized services that rideshare companies cannot provide, such as bariatric transport or long-distance medical trips.
  3. Community Partnerships: Forge strong relationships with healthcare providers, social services, and community organizations to become the preferred transportation partner for complex patient needs.

The Future of NEMT

The NEMT industry is projected to expand at a compound annual growth rate (CAGR) of 8.7% through 2028. indicating a bright future for providers. This growth is driven by an aging population, increasing prevalence of chronic diseases, and a growing recognition of transportation's role in healthcare outcomes.

Conclusion

While rideshare companies have entered the healthcare transportation space, NEMT providers should not fear this development. By focusing on their unique strengths, embracing technological advancements, and exploring collaborative opportunities, NEMT providers can continue to play a crucial role in ensuring access to healthcare for all patients, regardless of their mobility or medical needs.

The future of healthcare transportation lies in a diverse ecosystem where NEMT providers and rideshare companies coexist, each serving distinct patient populations and needs. By adapting to this changing landscape and continually innovating, NEMT providers can secure their position as essential partners in the healthcare journey.

Rideshare vs. NEMT Providers: Navigating the Future of Healthcare Transportation

Rideshare vs. NEMT Providers: Navigating the Future of Healthcare Transportation

Rideshare vs. NEMT Providers: Navigating the Future of Healthcare Transportation

The landscape of healthcare transportation is evolving rapidly, with rideshare giants Uber Health and Lyft Healthcare entering a space traditionally dominated by Non-Emergency Medical Transportation (NEMT) providers. This shift presents both opportunities and challenges for passengers, healthcare providers, and transportation companies alike.

The Rise of Rideshare in Healthcare

Uber Health and Lyft Healthcare have made significant inroads into the healthcare transportation sector, offering convenient and often cost-effective solutions for passengers needing rides to medical appointments. These services leverage their existing technology platforms to provide on-demand transportation, potentially reducing wait times and improving access to care.

Key Differences Between Rideshare and NEMT Providers

Service Level

NEMT Providers:

  • Offer comprehensive "door-to-door" or "door-through-door" service
  • Employ drivers trained in patient assistance, CPR, and first-aid
  • Provide specialized vehicles with wheelchair accessibility

Rideshare Services:

  • Typically offer "curb-to-curb" service
  • Drivers may lack specialized medical training
  • Limited availability of wheelchair-accessible vehicles

Cost Considerations

NEMT services often come at a higher cost due to specialized equipment and trained personnel. However, these costs are frequently covered by Medicaid or private insurance for eligible passengers. Rideshare options generally offer lower-cost rides but may not always be covered by insurance.

Passenger Suitability

NEMT providers are essential for riders with mobility limitations or those requiring assistance during transport. Rideshare services are better suited for ambulatory passengers with basic transportation needs.

The Impact on Healthcare Access

The introduction of rideshare services into healthcare transportation has the potential to significantly improve access to care. A study published in JAMA Internal Medicine found that the use of rideshare services for medical transportation resulted in a 7% reduction in missed appointments.

Challenges and Opportunities

While rideshare companies offer valuable options for addressing transportation insecurity, traditional NEMT providers continue to play a crucial role in serving passengers with complex needs. The future likely lies in a collaborative approach, where both sectors work together to offer a comprehensive range of transportation solutions.

The Growing Importance of Healthcare Transportation

As the U.S. population ages and chronic disease prevalence increases, the demand for reliable healthcare transportation is expected to grow. The NEMT industry is projected to expand at a compound annual growth rate (CAGR) of 8.7% through 2028.

Conclusion

The entry of rideshare companies into healthcare transportation presents both challenges and opportunities. While these services offer increased flexibility and potential cost savings, traditional NEMT providers remain essential for passengers with specialized needs. As the industry evolves, a collaborative approach that leverages the strengths of both models will be crucial in ensuring all passengers have access to the care they need.
