NEMT: Improving Access & Outcomes to Social Determinants of Health

NEMT: Improving Access & Outcomes to Social Determinants of Health

Non-Emergency Medical Transportation (NEMT): A Game-Changer in Addressing Social Determinants of Health

Non-Emergency Medical Transportation (NEMT) has emerged as a crucial tool in addressing social determinants of health, playing a pivotal role in improving healthcare access, promoting equity, supporting mental health, and enhancing overall health outcomes. This article explores how NEMT is revolutionizing healthcare delivery by tackling transportation barriers that have long impeded access to essential medical services.

Access to Care: Breaking Down Barriers

One of the most significant challenges in healthcare delivery is ensuring that patients can physically reach their medical appointments. NEMT breaks down this fundamental barrier by providing transportation for individuals who face challenges such as lack of personal transportation, financial constraints, or mobility issues. According to a study by the Medical Transportation Access Coalition, NEMT services resulted in an average monthly savings of $1,300 per person, highlighting its cost-effectiveness in improving healthcare access.1

Promoting Equity: Leveling the Healthcare Playing Field

NEMT plays a crucial role in promoting health equity by ensuring that everyone, regardless of their geographic location or disability status, can access medical appointments and essential services. This is particularly important for vulnerable populations, including the elderly, individuals with disabilities, and those living in rural areas. By providing reliable transportation, NEMT helps to level the playing field and reduce disparities in healthcare access and outcomes.

Supporting Mental Health: Beyond Physical Transportation

Transportation barriers can exacerbate feelings of isolation and contribute to mental health challenges, especially among older adults and individuals with chronic conditions. NEMT not only provides physical transportation but also fosters social connections and emotional well-being. Regular access to healthcare services, including mental health appointments, can significantly improve overall quality of life and reduce the burden of mental health issues.

Enhancing Health Outcomes: A Vital Link in Healthcare Delivery

By facilitating regular access to healthcare services, NEMT plays a vital role in preventive care, chronic disease management, and timely interventions. This consistent access to care ultimately leads to improved health outcomes and reduced healthcare costs. A study by the Transportation Research Board found that NEMT services are cost-effective or cost-saving for all 12 medical conditions analyzed, including prenatal care, asthma, heart disease, and diabetes.2

The Impact of NEMT on Healthcare Utilization and Costs

Research has consistently shown the positive impact of NEMT on healthcare utilization and costs:

  • A study suggests that NEMT more than pays for itself as part of a care management strategy for people with chronic diseases, resulting in a total positive return on investment of over $40 million per month ($480 million annually) per 30,000 Medicaid beneficiaries.1
  • NEMT has been deemed cost-effective and cost-saving for preventive care (e.g., dental care, screenings for breast and colorectal cancers) and the treatment of chronic conditions.1
  • Beneficiaries who used NEMT services for scheduled medical appointments based on prescribed treatment regimens enjoyed better health and incurred significantly lower long-term costs than those who didn't maintain regular appointments.1

The Future of NEMT: Technology and Innovation

As the NEMT industry continues to evolve, technological advancements are playing a crucial role in improving service delivery and efficiency. Innovative NEMT software solutions, such as Bambi, are revolutionizing the way NEMT providers manage their operations. These platforms offer features like automated scheduling, real-time tracking, and seamless communication, enhancing the overall NEMT experience for both providers and patients.

Conclusion: NEMT as a Cornerstone of Healthcare Access

Non-Emergency Medical Transportation has proven to be a game-changer in addressing social determinants of health. By breaking down transportation barriers, promoting equity, supporting mental health, and enhancing overall health outcomes, NEMT is playing a crucial role in improving healthcare access and reducing disparities. As healthcare systems continue to recognize the importance of addressing social determinants of health, investing in and expanding NEMT services will be essential for creating a more equitable and effective healthcare system.

For healthcare providers and policymakers, embracing NEMT as a key component of healthcare delivery is not just a matter of convenience—it's a strategic imperative for improving population health and reducing healthcare costs. By leveraging the power of NEMT, we can create a healthcare system that truly serves all members of our communities, regardless of their transportation challenges or socioeconomic status.

How to Screen NEMT Drivers | NEMT Experts P E47

Bambi CEO Nirav Chheda talks with Pamela Mack, the Owner and President of Occuscreen, about how to conduct screening, background checks, and drug testing in the process of hiring NEMT drivers.

Podcast Show Notes:

Summary

This podcast episode features a discussion between Nirav Chheda and Pamela Mack, owner of Occuscreen, about the importance of screening NEMT (Non-Emergency Medical Transportation) drivers. Pamela explains that her company has been in the background and drug screening business for over 30 years, with 15 years of experience specifically in the NEMT industry.

The conversation covers the types of screening required for NEMT drivers, which typically include criminal background checks, sex offender registry checks, motor vehicle reports, and drug screenings. Pamela emphasizes the importance of following regulatory guidelines, particularly the Fair Credit Reporting Act (FCRA) and Equal Employment Opportunity Commission (EEOC) guidelines, to avoid legal issues and protect both drivers and NEMT businesses.

Pamela provides insights into the screening process, explaining that it's not as instantaneous as many people believe. She notes, "There's actually different processes, different courthouses we sometimes have to reach out to. It's not by Social Security number, it's by name and date of birth and then addresses and all sorts of other ways that we identify someone." The typical timeline for completing a screening is 1-3 business days, depending on the state's requirements.

The episode also addresses common misconceptions about background checks, such as the difference between fingerprinting and criminal background checks. Pamela advises that unless specifically required by contract, fingerprinting is often not the best method for employment screening.

Pamela stresses the importance of working with an experienced and accredited Consumer Reporting Agency (CRA). She recommends that NEMT businesses choose a screening company that is accredited by the Professional Background Screening Association (PBSA) and offers good customer service. As she puts it, "Find a partner that will actually work with you and answer your questions and be there for you. And then I think you'll find it's not that difficult, it's not that scary, it's not overwhelming."

Overall, the episode provides valuable insights for NEMT businesses on how to navigate the driver screening process effectively and compliantly.

Key Takeaways:

  1. Simple Onboarding Process: The onboarding process for background checks and drug screenings at Pamela Mack's company is straightforward. After credentialing, transportation providers (TPs) can easily log in and manage background checks and drug screenings electronically.
  2. Detailed Screening Process: Background checks are not instant and rely on multiple steps, including courthouse verification. Drug screenings are similarly processed, often involving QR codes and electronic consent forms.
  3. FCRA and Compliance: It's essential to follow legal guidelines such as the Fair Credit Reporting Act (FCRA) and EEOC recommendations, especially when handling sensitive information like criminal records or drug test results.
  4. Fingerprinting vs. Background Checks: Fingerprinting is often misunderstood as the best background check method. Mack emphasized that fingerprinting often shows arrest information, not convictions, which is less helpful for hiring decisions.
  5. Legal Risks of Non-Compliance: Non-compliance with regulations, such as obtaining written consent for background checks, can expose companies to significant legal risks, including class-action lawsuits.
  6. Service Across the U.S.: Mack’s company operates across the U.S., working with both large and small transportation providers, offering tailored solutions to meet specific NEMT screening needs.

Key Quotes:

  1. On the Process:
    • Pamela Mack: “Once you’ve done it once, you’re set up, and you look through the instructions or call us for help, it’s actually a pretty simple process.”
  2. On Background Checks:
    • Pamela Mack: “Most people think a background check is all about a social security number. But it's not instant. It requires names, birth dates, addresses, and multiple sources.”
  3. On Fingerprinting:
    • Pamela Mack: “Unless your contract requires fingerprinting, we would never recommend it. It mostly pulls up arrest information, and arrest records can’t be used for hiring.”
  4. On Compliance Risks:
    • Pamela Mack: “If you ignore those things, you’re opening yourself up to a lot of liability... Do it right instead of finding out the hard way what you did wrong.”
  5. On Choosing a Screening Partner:
    • Pamela Mack: “Find a partner that will actually work with you, answer your questions, and be there for you.”

Part 4: Secure Funding in California

Starting a startup in California isn't for the faint of heart, however, it sure is one for those Bambi-typed souls! Beginning a biz venture in Cali is full of exciting possibilities, but financial planning is key. Understanding the initial and ongoing expenses is imperative; think of them as your California keys to success! Don't hesitate to discuss these issues your potential biz partners and your family — they'll be your support system on this wave of a journey.

From buying vehicles and covering licensing fees to managing insurance premiums, marketing expenses and day-to-day operations, startup costs can quickly add up. Once you've mapped out these financial needs, securing the right funding is essential for launching and maintaining your NEMT business to bring in more gold into the Golden State.

Consider exploring options like small business loans, connecting with angel investors, or utilizing crowdfunding platforms to gather the necessary funds. Here are some steps and resources to kickstart your search for financial support in California:

1 | Small Business Loans

The SBA offers various loan programs for small businesses, including the 7(a) loan program, which provides financial assistance for businesses with special requirements.

California Capital Access Program (CalCAP) and California’s Infrastructure and Economic Development Bank (IBank) both are great resources that help small businesses secure loans and support with financial planning.

2 | Financial incentives

California Governor's Office of Business and Economic Development (GO-Biz) oversees economic development initiatives and administers various incentive programs aimed at attracting and retaining businesses in California.

3 | Grants

While direct grants tailored specifically for launching new businesses are uncommon, there are opportunities available for specialized purposes such as technological innovation and rural development. If you offer NEMT services to rural residents in California, you may qualify for grants listed on Grants.gov, a federal resource for grants. For additional opportunities, check out California-specific small business grants on the US Grants organization website here.
