  1. Download the Application: Visit the Tennessee Carriers website and locate the "NEMT" section. There you will find a link to download the "Prospective NEMT Transporter Information Form."  
  2. Complete the Application Thoroughly: Fill out all sections of the application form accurately and completely. Pay special attention to ensuring the accuracy of the State Specialty Code, which can be found in the list provided using your Taxonomy Code.  
  3. Submit the Application: Once completed, send the application form via email to contracts@tenncarriers.com. Alternatively, you can fax the form to (901) 339-2182.  
  4. Await Review and Selection: Tennessee Carriers will review your application. Note that not all applicants will be accepted. Selection is based on network needs, provider capabilities, experience, and location.
